Office Hours
Our telephones are staffed from 8:30AM until 5:00 PM continuously on Monday through Friday, and from 8:30AM until 12:00PM on Saturdays. Patients are seen by appointment only. After hour sick appointments on Saturdays will be charged an extra fee of $30.00.
Insurance
Effective January 1,2025, we are NOT contracted in the PERS GOLD netwo through Blue Shield.
We accept most PPO insurance plans including; Blue Cross, Blue Shield,Aetna, Cigna, Health Net, Motion Picture, SAG, Director's Guild, Writer's Guild and United Health Care.
We are NOT contracted with Medi-cal, Multiplan and First Health.
HMO's: We are contracted with Regal/Lakeside Medical Groups

When you make your first visit to our office we will ask you to complete a few "Patient Information Forms". For your own convenience, and to save you time in the office, you may wish to download and complete the "Family & Child's Health History" and "Patient Information - Insurance & Legal" forms before your appointment.
